To ensure we can provide you with the best possible care from our very first meeting, we'll send you some important paperwork via email. These forms help us understand your needs and explain everything about our work together – from your rights as a client to our clinic policies. We ask that you complete these forms at least one week before your first appointment.

A few practical details: we'll need to verify your insurance coverage and keep a credit card on file (this helps us manage appointment commitments). While we know paperwork isn't the most exciting part of starting treatment, having everything in place before we meet allows us to focus on what really matters – supporting you on your path to wellness.

Preparing for Your First Visit

We want your first session to be the start of a supportive partnership in your mental health journey. To make our time together most effective, please bring:

  • Your insurance card
  • Your current medications (either a list or the bottles themselves)
  • Signed Release of Information forms if you'd like us to coordinate with other healthcare providers

A few helpful tips to ensure a smooth first visit:

  • Please arrive 10 minutes early to get settled – this helps us make the most of our time together. (Note that we consider appointments missed if you arrive more than 15 minutes after the scheduled start time.)
  • For our younger clients (under 18), we require at least one parent or guardian to attend the first session, though having both parents present is ideal when possible.

Our office currently has openings on Monday thru Thursday from 9:00AM to 5:00PM.

Provider-Patient Relationship

Justin Rice, PMHNP practices psychiatry and maintains a professional and therapeutic relationship with his clients. Justin and his clients work together towards meeting aligned treatment goals to achieve recovery. Due to the nature of psychiatric treatment, and to ensure that your treatment with Justin is going well, patients must be seen a minimum of at least once every three months to be considered an active patient and to receive medication refills. While medication is being adjusted clients need to come in more frequently.

Justin is currently in-network and accepting the following insurance plans:

  • Regence/Blue Cross Blue Shield
  • MODA (formerly ODS)
  • Pacific Source

As a courtesy to our clients Sakura Recovery and Wellness, LLC. will bill your insurance company on your behalf. It is recommended that you contact your insurance company to verify your mental health benefits. It is a client’s (your) responsibility to determine your deductible and copayment/coinsurance. Full payment of your deductible and/or copayment/coinsurance is due at the time of your appointment.

We do not bill out-of network insurance plans.

If you have an immediate life-or-death emergency call 911 or go promptly to the emergency room for assistance.

Our office does not utilize live calling but we will respond to voicemails and emails within 24 hours. Shari, office manage will pass all relevant messages to Justin. We make a strong effort to return patient calls on the day received or the next day we are in the office. Please make sure we have your current home, work, or cellular number on file. If you have an urgent need please make this clear so that we do not underestimate your concern. Please don’t hesitate to call back.

The routine practice of our office is to electronically submit a prescription to cover your needs until your next appointment.  There should be no need for additional refills if you keep scheduled appointments or reschedule promptly.  If an exception occurs, please call us at least 3 working days before you will run out for a refill authorization.  Refills will be approved only for patients with a scheduled follow up appointment and on a case by case basis. Typically Justin will do this for you as a one-time exception, but after that, if medications are refilled without appointment (if at all), they will be only enough to last until the next scheduled appointment. This may result in you having higher copay for your medication. Controlled substances cannot be refilled by telephone and will only be rewritten during your next appointment.

All co-payments, coinsurance and/or deductible payments are required at the time of your appointment. We accept cash, money orders, and most major credit and debit cards.

There is a charge for appointments missed, cancelled, or changed with less than a two business days’ notice. Please notify us promptly if you are not able to keep your appointment so we may contact someone on the waiting list.  If you miss three appointments without notification you may be subject to dismissal as a patient. We don’t want to do this! So please reach out by email, phone, or leave a message on our secure voicemail.

Due to the additional time and costs incurred, there is a charge of $25 for record copies, and extended or complex phone calls. Letters, reports, disability paperwork, medication authorizations, or extended calls made to third parties on your behalf will also be subject to a charge based on an hourly rate. These charges are generally not covered by insurance.
